Apothecare is a team of pharmacists serving North Mississippi specifically trained to collaborate with prescribers in their own practice. Our pharmacists deliver value-based services directly to patients to equip them with the confidence and knowledge to regain control of their health by enhancing patient outcomes. We are committed to serving the unique needs of each prescriber to maximize our impact in the practice. By focusing on wellness and preventative services, prescribers are able to offer more frequent care to their patients who need it most and also benefit from new streams of revenue.

Our pharmacists maximize the practice’s MIPs scores as many of those outcomes are achieved from our targeted approach during the AWV.

RPM is an effective addition to patients receiving CCM. Real time lab results transmitted electronically helps to improve how your practice tracks the progression of patients with chronic conditions. Our pharmacists review the data daily to identify outliers to prevent costly adverse events rather than treating the consequences after they occur.

Transitional Care Management

Transitional Care Management

Increase contact with your patients post discharge when they’re most vulnerable. Our pharmacists contact these patients after discharge to confirm medication changes, review any referrals and schedule important follow up appointments with the practice. These patients then receive a weekly check-in call to ensure adherence to their medications and to identify possible adverse events, that if prevented, avoid readmission to the hospital.
